You can have your name legally changed and request a new ss number. It's a real pain but would effectively change your identity for the purpose. You'll probably need to show a need (safety, etc.) for changing both but it can be done. Sorry, I wouldn't know which to do first though. Also make sure the court records of changing your name are sealed so that you can't be found that way (by having your new name discovered).

I don't know but am sure it can't be too easy. Maybe talk to a legal person and they could tell you the necessary steps to take.
If your ex is causing you hassle, DON'T HESITATE in reporting him to the police. Bullies like that have to be stood up to.
